Primary Care Sports Medicine Fellowship
at the University of Florida

The University of Florida has a history of outstanding athletics and a reputation for excellence in medicine. The Primary Care Sports Medicine Fellowship will combine elements of both to create an outstanding educational experience for the sports medicine fellow. The fellowship will be housed mainly in the UF Student Health Care Center (SHCC) which provides non-operative sports medicine and primary care for the approximately 50,000 students of the University of Florida including its athletic teams. DCHFM physicians, both at SHCC and the residency program, will play an integral role in educating the fellow. The faculty includes 4 CAQ primary care sports medicine physicians and 6 physicians who are current or previous team physicians at the University of Florida. Other faculty members are from various departments at the university including CHFM, Orthopedics, Sports Health, Athletic Training, Physical Medicine and Rehabilitation, Cardiology, Radiology, Nutrition, Psychology, Physical Therapy, Anatomy, and Pharmacy. With the addition of the primary care sports medicine fellowship, the DCHFM looks to continue its long legacy of excellence in family medicine education. 

In January 2007 the ACGME granted preliminary accreditation to our primary care sports medicine fellowship. The program will be 12 months in duration and enroll 2 fellows. Our start date will typically be mid July. Applicants may be from all primary care fields but preference will be given to family medicine physicians. 

Application Process

Applications for the 2009-2010 fellowship year will be accepted until the AMSSM universal deadline of October 1, 2008. The universal fellowship application form will be used and may be found at this website:

We will accept either paper applications mailed to the address below or will accept applications via email. If submitting by email, download and fill out the application form and save it. Attach the saved form to an email sent to Chari Holder, Program Assistant at chari@ufl.edu

In addition to this form and its requested documentation, please submit copies of the first page of all publications and presentations listed in your application.

We are currently in the process of joining the national resident matching program and expect to take part in the match starting fall 2007.
